4/7/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Client was focused, paying attention, and on task. Client displayed cooperative behaviors toward authority and appeared to be in a positive mood, as evidenced by her straight forward expressions and pleasant demeanor. QMHP-C engaged mother in a discussion to determine behaviors since the last session. Mother reports client struggles with learning how to resolve conflict in a healthy manner as she likes to fight. 2. Behavior: Client’s guardian communicated with counselor that she likes the idea of engaging in role play with Client because she feels Client would better understand negative versus positive. 1.Intervention: QMHP-C monitored Client individualized behavior modifications during therapy to see if Client needed assistance. QMHP-C expressed gratitude toward positive interaction with mom and mom’s boyfriend. QMHP-C encouraged Client to continue displaying positive behaviors. QMHP-C supported Client involvement in selected activities. QMHP-C processed with Client about what she learned from activity. QMHP-C praised Client for her cooperation despite not wanting to have session today. QMHP-C discussed with Client how empathy, communication, and awareness of another point of view can prevent or reduce conflicts. QMHP-C brainstormed with Client a list of conflicts that typically occur in her home in efforts to identify possible conditions or triggers that preceded the actual conflict. 2. Intervention: QMHP-C utilized role playing to demonstrate the calming effects of predicting the thoughts, feelings and actions of self and others prior to the outbreak of a conflict. QMHP-C prompted Client to share anticipated thoughts and feelings and to indicate how this process reduces angry reactions. QMHP-C asked appropriate questions about group activity. QMHP-C reviewed ISP goals and objectives. QMHP-C reminded Client of home rules and expectations especially during quarantine. QMHP-C supported and monitored Client in social and academic setting to ensure she was gaining insight on her mental illness. 1.Response: Client did not require assistance at this time. The family was acceptive of the gratitude QMHP-C showed to them, giving the same respect back and was very supportive. Client modeled different ways to walk away from a conflict when frustrated. Client modeled how to separate herself from negative situations by counting to 10. Client demonstrated how to verbalize her feelings without yelling and being aggressive. Client was able to speak calmly and demonstrate her improved verbalization skills while conversing with mom. Client participated in brainstorming exercise and was able to identify possible conditions or triggers that precede a conflict at home. Client did not display any outbursts during the role play exercise. Client had difficulty understanding due to minor distractions. Client appeared to be learning from her mistakes and correcting them throughout the day. Client responded well to praise. Client was receptive as QMHP-C demonstrated and defined the meaning of empathy. 2. Response: Client actively participated with QMHP-C as they discussed and processed empathy. Client demonstrated that she was able to process productive expressions of anger. Client displayed active behaviors in home setting. Client is making progress with expressing herself. Client made insightful comments which contributed to discussion. Client did not leave session or get frustrated like she normally does when things don’t go her way. Client said she likes creative and artistic activities. Client agreed to follow instructions and finish her work. Client was seen interacting well with counselor and parent. Client was observed working towards ISP goals and objectives. Client seemed to be enjoying the session and looked excited when transitioning back to normal activities.
